Post-Acute Care Services
We’re here to address the care environment for all patients following discharge from BMH and other acute care facilities. Our team of clinicians will be involved in direct patient care from the time of acute care discharge to admission at a skilled nursing facility.
We follow best practices for managing medical problems in post-acute medical episode recovery/rehabilitation. Our goal is to stabilize chronic conditions so patients can return home. Our team also supports decisions made at the end of life for unique care directed toward comfort and respect for the patient in the dying process.
Post-Acute Care Services will provide leadership in efforts to:
- improve patient safety
- prevent hospital-acquired infections
- manage care transitions
- provide compassionate palliative care at the end of life
- improve health care technology (for example, electronic medical records and computerized physician order entry systems).
